Where do I find the approval page form?

I've heard I need to have my advisor sign the library's approval form. Where can I find it?


Answer

As of December 12, 2022, the library does not require a signature page for deposit. Your manuscript will include a set of preliminary pages that are outlined in the library's format guidelines; the third page of your document is an approval page that includes the title, author, manuscript statement, month/year that the submission was approved, and the names of your primary advisor(s), program executive officer or director, and supervisory committee (if applicable). See sample pages for further details.

The manuscript statement should read:

This manuscript has been read and approved for the Graduate Faculty in Psychology in satisfaction of the dissertation requirement for the degree of Doctor of Philosophy, The City University of New York.

Note: Be sure to edit the highlighted areas of the statement to match your submission details. For example, a master's capstone project in Data Analysis and Visualization would read:

This manuscript has been read and approved for the Graduate Faculty in Data Analysis and Visualization in satisfaction of the capstone project requirement for the degree of Master of Science, The City University of New York.
